Program Focus

YOFAK implements Global Fund TB and Malaria projects in Homa Bay County

Global Fund -Amref Health Africa in Kenya – Tuberculosis Programme:

  • – Support Community Health promoters (CHPs) to visit households of bacteriologically confirmed of DS-TB patients for health education and referral.
  • – Support Health facilities to conduct Program Quality and Efficiency– Conduct Organizational Development and Systems Strengthening (ODS) and mentorship to Civil Society organizations– Support national and county champions to conduct media engagement and Mobilization

     

  • – High Level Engagement of National and County Legislators through community led social mobilization campaigns to include domestic resources

     

  • – Facilitate CHVs to conduct tracing of TB treatment interrupters (3% will be lost to follow up if not traced.

Global Fund – Amref Health Africa in Kenya Malaria Programme

 

  • – Community Case Management of malaria (CCMm)
  • – Uploading of CHU data onto HMIS
  • – Community Health Units ( CHUs)  Support Supervision
  • – Annual Quality Assurance for mRDT use at the community level by CHVs.
  • – Support Coordination of CHU activities by CHEW/CHA
  • – Training of Community Health Assistants on Community Cases Management of malaria & Malaria in Pregnancy ( MIP)
  • Annual county and sub county data and performance review meetings for CCMm
  • – Engage CHVs to identify IPTp missed opportunities for referral to ANC

HIV prevention, Treatment and care Programmes

HIV Testing Services ( HTS) training
  • The institution is accredited by NASCOP to offer
Youth Sexual Reproductive Health Services

Social mobilization on use of Combination HIV Prevention implemented within Biomedical, Behavioral and Structural frameworks to Reduce New HIV Infections

Behavioral strategies

Social mobilization on Sexual debut delay, Sexual partner reduction, Consistent condom usage, HIV counseling and testing, Sexual abstinence

Biomedical intervention

Promotion of uptake and consistent usage, adherence to harm reduction strategies and decreasing Substance use.

The organization employs Strategies to reduce HIV risk exposures, transmission and/or infection by making Male and female condom available and accessible in various outlets

Structural Interventions

The organization focuses on Sex behavior, Condom use, multiple partners risk reduction, Unprotected Sex, ABCDE around the principles of (ABCDE) that is: abstain, be faithful, use a condom, disclose and empower. Linkage to care, Abstinence/sex initiation, Testing/Treatment.
